APPLICATION REQUIREMENTS

Please read the following information and subsequent Application Form carefully. Answer the questions required and DO NOT include a curriculum vitae. Extensive curricula vitae, research proposals, etc., will be disregarded

Note that the completed application and all letters must be in the office of the Secretary by January 15^th for fellowships starting in July of that year. Facsimiles of any materials are not accepted.

APPLICATION CRITERIA

1. Applicants must be graduates of an accredited medical school or school of osteopathic medicine.
2. Applicant must complete their fellowship within 3 years of residency
3. The fellowship must be conducted in the United States
4. A minimum of 20% of the fellowship must be spent in research

REQUIREMENTS OF ALL APPLICANTS

1. A statement setting forth the applicant's major opthalmic interest, professional aims and objectives, and noting qualifications deserving of particular consideration - limit one page. (can be copied - pasted from a word document)
2. A letter of recomendation from the Chair of Ophthalmology who supervised residency training
3. A letter from the preceptor of your fellowship program describing your fellowship program, including a statement that a minimum of 20% of fellowship will be spent in research.
4. Two (2) additional letters of recommendation from physicians who have supervised the applicant.
5. A letter of acceptance of applicant at institution where fellowship is to be taken (this may be a copy of the applicant's match letter).
6. A letter of recommendation from the Chairman of the department who has offered an academic appointment (optional).
7. DO NOT SEND CV's. Place all information on application and in personal statement

PLEASE NOTE: Facsimiles of application materials or letters of recommendation will not be accepted. Any information mailed within one week of the deadline should be mailed using overnight or second day service.

DEADLINE FOR SUBMISSION OF ALL APPLICATIONS AND MATERIALS: January 15^th

January 15^th is the deadline for fellowships beginning July 1^st of that year. Call one week before the deadline to determine that your application is complete. The phone number is 216.445.8145. This is the applicant's responsibility. A certificate of completion will be sent to the applicant upon completion of the fellowship. Change of address during/after fellowship period should be forwarded as soon as possible.
